Tips and strategies for addressing the taboo of bad breath

It is one of the last great taboos of our society: bad breath (medically: halitosis). Those affected often suffer in silence, withdraw socially, or develop great insecurities in dealing with others. Shame is out of place here. Bad breath is not a sign of poor hygiene, but usually a medical imbalance that can be treated very well.

As dentists, we would first like to clear up a persistent myth: bad breath almost never comes from the stomach. In about 90% of cases, the cause lies directly in the oral cavity. That’s the good news – because that’s exactly where we can start together.

To solve the problem, you have to understand the mechanism. Bad breath is caused by anaerobic bacteria (bacteria that live without oxygen). These microorganisms decompose proteins from food residues, saliva, or dead cells. This produces so-called “volatile sulfur compounds” as a waste product. It is precisely these sulfur compounds that we perceive as unpleasant.

To solve the problem, one must understand the mechanism. Bad breath is caused by anaerobic bacteria (bacteria that live without oxygen). These microorganisms decompose proteins from food residues, saliva, or dead cells. This process produces so-called “volatile sulfur compounds” as a waste product. It is precisely these sulfur compounds that we perceive as unpleasant.

These bacteria hide in niches that are protected from oxygen: in deep gum pockets, under leaking crown margins, and especially in the deep furrows of the back of the tongue.

Your 5-step plan for fresh breath

If you feel you are affected, you can immediately integrate the following measures into your routine:

1. The No. 1 bacterial reservoir: The tongue

About 60% to 80% of the bacteria that cause bad breath live on the tongue – more specifically, on the back third.

  • The measure: Use a tongue cleaner or tongue scraper daily (ideally in the morning). Toothbrushes are often too soft for this and also quickly trigger a gag reflex because they are too bulky.
  • Technique: Gently pull the scraper from the very back to the front. If you don’t have a scraper at hand, an upside-down teaspoon also works as a temporary solution.

2. Use chemistry: Zinc instead of alcohol

Many patients reach for harsh mouthwashes. Caution: if the rinse contains alcohol, it dries out the oral mucosa, which worsens the odor in the long term.

  • The measure: Look for the ingredient zinc in toothpaste and mouthwashes. Zinc ions have the chemical property of binding sulfur compounds and thus neutralizing the odor instead of just masking it.

3. Niche cleaning: Interdental care

Since bacteria love low-oxygen niches, the spaces between the teeth are their perfect hiding place.

  • The measure: The daily use of dental floss or interdental brushes is non-negotiable for halitosis. They remove the “food” for the bacteria and bring oxygen into the spaces, which inhibits bacterial growth.

4. Saliva flow is the natural enemy of odor

Saliva is rich in oxygen and has a natural rinsing function. A dry mouth (xerostomia) – for example, due to stress, speaking for a long time, or medication – almost automatically leads to bad breath.

  • The measure: Drink enough water or unsweetened herbal tea (green tea contains polyphenols that can inhibit bacteria). Chew sugar-free gum (with xylitol) after eating to stimulate saliva flow.
  • Home remedies: Chewing fresh parsley, mint, or a slice of ginger has a short-term neutralizing effect and also stimulates saliva.

5. The professional check-up (halitosis consultation)

If home remedies do not help permanently, there is often a cause that you cannot fix yourself. These include unnoticed periodontitis (gum inflammation), leaking filling margins, or hidden sources of inflammation.

  • What we do: In a special examination, we objectively measure the breath and inspect the oral cavity for bacterial niches. Often, a targeted professional teeth cleaning (PTC) in combination with periodontitis therapy brings immediate relief, as this cleans the deep pockets that you cannot reach at home.
